Harmony Roots Wellness, INC d/b/a Harmony Roots Wellness (“we,” “us,” or “our”)

This policy explains how we collect, use, and share information through our website, our text messages, and our emails. It does not cover health information in your medical record. That information is protected by federal and state health privacy laws, and our Notice of Privacy Practices explains how we use and share it.

1. Information We Collect

• • Information you give us, such as your name, preferred name, pronouns, mobile phone number, email address, and the contents of forms or messages you send us.
  • Scheduling information, such as appointment requests and the dates and times you choose.
  • Communication preferences and consent records, including which messages you agreed to receive and when.
  • Website usage information, such as device type, browser, and pages visited.

2. How We Use Your Information

  • To schedule, confirm, and manage appointments
  • To send appointment reminders and clinic updates you have agreed to receive
  • To send information about clinic events and services, including services that have a fee, only if you have agreed to receive marketing messages
  • To operate and improve our website

3. Sensitive Information

          Some information you may share with us, such as gender identity, sexual orientation, or pronouns, may be considered sensitive                 under Colorado law. We ask for it only if you choose to provide it, and we use it to address you respectfully and to provide care.

4. Text Messages and Email

         We send two kinds of messages, and you choose each one separately.

• Appointment confirmations: reminders, and clinic updates about your care, sent only if you have agreed to receive them.
• Event and promotional messages: announcements about clinic events, groups, and services, including services that have a fee, sent only if you have separately agreed to receive marketing messages. Agreeing is not required to receive care or to purchase services.
• We do not sell or share your SMS opt-in data or personal information with third parties for marketing purposes. We do not sell, rent, or share your mobile number, email address, or consent records with third parties or affiliates for their own marketing. We do not base event or promotional messages on your health information or on the type of care you receive.Text messages are not a fully secure method of communication. Please do not send health details, medical records, or urgent concerns by text or email.
• Email. Our emails about events and services are clearly identified as promotional when they are marketing. They include an accurate subject line and our physical mailing address. You can unsubscribe from marketing emails by emailing us and we will do our best to honor your request within 10 business days.

5. How We Share Information
   

   We do not sell your personal information. We share information only with:

• Service providers that help us run our practice, such as our electronic health record and scheduling platform, text messaging provider, email provider, payment processor, and website host. These providers must protect your information and use it only to provide services to us.
• Others when required by law, such as in response to a court order, or to protect safety as permitted by law.
• Successors in a business transfer. If our practice is sold, merged, or reorganized, your information may transfer as part of that change. We will notify you if this happens.

6. Health Information

        Health information we create or receive in providing care is protected health information (PHI). We protect PHI as required by HIPAA and Colorado law. See our Notice of Privacy Practices for details. 

7. Data Retention

We keep website, text, and email information only as long as needed for the purposes described above or as required by law. Medical records are kept for the periods Colorado law requires. You may ask us to delete your text message and email data at any time, though some records must be kept for legally required periods.

8. Your Choices

• Text messages: reply STOP to stop messages, or contact us.
• Email: contact us at Office@HarmonyRootsWellness.Org.
• Preferences: change which messages you receive by contacting us at 970-716-4220 or Office@HarmonyRootsWellness.Org.
• Access, correction, or deletion: request access to, correction of, or deletion of your personal information by contacting us. Colorado residents may have additional rights under the Colorado Privacy Act. 

9. Security

We use reasonable administrative, technical, and physical safeguards to protect your information. No method of transmission or storage is completely secure.

10. Children

This website is not directed to children under 13, and we do not knowingly collect personal information from them through this website.

11. Changes to This Policy

We may update this policy. The effective date above shows the most recent revision.
