Affordable Healthcare for Every Body

Medicaid, Community Pay Scale & Transparent Pricing at Harmony Roots Wellness

Fort Collins, Colorado | Serving Northern Colorado

Healthcare shouldn’t depend on your income, your insurance, your identity, or whether you’ve felt welcome in a medical office before.

At Harmony Roots Wellness, we believe healthcare is a basic human need. Everyone deserves a provider who listens, explains, and partners with them in their care.

Harmony Roots Wellness is a Colorado charitable organization governed by a volunteer Board of Directors and operated as a not-for-profit healthcare clinic. Our mission is to remove barriers to quality healthcare—not create them.

We have applied for federal 501(c)(3) tax-exempt status and will continue pursuing that designation. Regardless of our tax status, every decision we make is guided by one question:

How can we make compassionate healthcare more accessible for our community?

Whether you’re seeking gender-affirming hormone therapy, gynecologic care, menopause treatment, contraception, primary care, wellness services, or medical weight management, we are committed to making care as affordable and accessible as possible.

We Accept Colorado Medicaid

Harmony Roots Wellness proudly accepts Colorado Medicaid for many of our healthcare services.

Finding providers who accept Medicaid—especially for LGBTQIA+ healthcare, gender-affirming hormone therapy, gynecology, or trauma-informed care—can be challenging. We believe Medicaid patients deserve the same time, respect, and individualized care as anyone else.

If you’re unsure whether your visit or treatment is covered, our team is happy to help verify your benefits before your appointment.

Our goal is to help you understand your options—not surprise you with unexpected costs.

Our Community Pay Scale

We know that financial circumstances aren’t always the same from month to month.

Unexpected medical bills, changes in employment, caring for children or aging parents, or simply the rising cost of living can affect anyone.

Rather than asking patients to prove hardship through paperwork and financial documentation, Harmony Roots Wellness has chosen a different path.

We choose trust.

Our Community Pay Scale is a recommended payment model based on household size and annual income. It offers guidance for choosing a payment amount that reflects your current financial circumstances while helping sustain affordable care for everyone in our community.

Unlike many traditional sliding-scale programs, our Community Pay Scale is based on the honor system.

You are never locked into one payment tier.

If you’re experiencing a temporary hardship, you’re welcome to choose a lower tier. If your circumstances improve and you’d like to contribute at a higher level, you’re welcome to do so.

Healthcare shouldn’t become another source of stress.

How the Community Pay Scale Works

We’ve intentionally kept the process simple.

Step One

Review our recommended payment tiers, which are based on household size and annual income.

Step Two

Choose the payment tier that best reflects your current circumstances.

Step Three

When you check in for your appointment, we’ll simply ask:

“Which payment tier would you like to use today?”

That’s it.

No tax returns.

No pay stubs.

No financial interviews.

No judgment.

Just compassionate healthcare built on mutual trust.

Why We Trust Our Patients

Healthcare works best when relationships are built on respect.

We know that no chart, tax return, or income statement tells the whole story.

Some months life is comfortable.

Some months it isn’t.

By trusting patients to honestly choose the payment tier that reflects their circumstances, we’re able to spend less time processing paperwork and more time caring for people.

Patients who are able to contribute at higher payment tiers help make healthcare possible for neighbors experiencing financial hardship.

Together, we’re building a healthier, more compassionate Northern Colorado.

Transparent Pricing

We believe you should know what healthcare costs before you receive it.

Whenever possible, we’ll discuss expected costs, insurance coverage, laboratory fees, and treatment options before services are provided.

If additional testing or procedures become necessary during your care, we’ll explain your options so you can make informed decisions.

No surprises.

No pressure.

Just honest conversations.

Insurance and Self-Pay

We welcome patients who are:

  • Covered by Colorado Medicaid
  • Paying out of pocket
  • Using our Community Pay Scale
  • Using Health Savings Accounts (HSA)
  • Using Flexible Spending Accounts (FSA)
  • Seeking services not covered by insurance

If you have questions about payment options, we’re always happy to help.

Why Harmony Roots Is Different

Healthcare should feel like someone is on your side.

That’s why we intentionally provide care that is:

  • LGBTQIA+ affirming
  • Gender-affirming
  • Trauma-informed
  • Neurodivergent-friendly
  • Weight-inclusive
  • Disability-conscious
  • Relationship-centered
  • Evidence-based

Many of our patients tell us they came to Harmony Roots after years of feeling dismissed, rushed, or unheard elsewhere.

We’re honored to provide a different experience.
